1. Site Plan- preferably the original copied down to an A 4 paper by emailing. (Do not send original floor plans.) This floor plan needs to identify the interior rooms. Add to this, exterior elements including roads, surrounding buildings, bodies of water, swimming pool, power lines, other major obstructions or views like the mountains, etc. NOTE: If you do not have a floor plan, you must draw one to scale.
2. Identify key areas that need to be addresses specifically; money situations, health problems, conflicts, etc. Advise any other questions or concerns you may want addressed as well.
3. Provide your address (home or office), phone, fax, mobile numbers and e-mail address.
4. Payment are taken at the time of visit to site in cash.
Its best that all the above information is sent prior to the date of the visit so as to enable the practitioner to prepare the case details prior to visit.
